Transforming services for disabled children, children with palliative care needs and their families is a high priority for NHS West Kent. This cannot be achieved without working closely with disabled children, their parents / carers, professionals and key partners such as Kent County Council.
The Department of Health, as a result of the Parliamentary Hearings on Services for Disabled Children and their Families and the subsequent publication of Aiming High for Disabled Children: Better Support for Families and Better Care: Better Lives, has asked all Primary Care Trusts in partnership with their Local Authority to transform services providing the following:
- Short Breaks
- Community Equipment
- Wheelchairs
- Palliative care / end of life care
